I am trying to use the LSF security for a very simple class - The users will only be able to access one form.
I thought that I would not have to set up a UNIX account if the users were to use Portal only and only an online form, but so far no luck. I get this that says "portal cannot load because the profile service returned an error". ANy ideas where to start looking?

It depends on the form. If it is a batch form, you will need to setup an underlying OS account.
There are a few online forms that also need an underlying OS account.
In either case, without seeing your setup, I'm making broad assumptions. With that understanding, the following are ideas that come to mind:
1) ALL users need some level of access to LOGAN and GEN. You will need to set that up explicitly for an LS-only user.
2) ALL users need access to a form's (TKN object) containers, such as PDL, CAT, and PGM objects.
3) When I have been "Portal cannot load" it is usually a global issue, and in that is a configuration issue. Unless you are sure someone can log into Portal without error, that is something that should be reviewed.

I'm comming at this from an Iseries point of view. In our situation all users that have access via portal must have a Iseries/UNIX account and their SSO ID of course. Our ESS users share that same Iseries/Unix account because of their limit access. But still they need that account.

You being in "Education" and me being in "Heatlhcare" maybe the same 'everyone moves around, has different jobs depending on the day' situation may not exist.
We've made the decision to give EVERYONE here an underlying Unix account.. That way our LS Sub-Administrators can control the access whether batch, or ESS only, simply by assigning groups.

Thank you everyone. It ended up being that I had not entered the product line on the environment form.
The suggestion of creating UNIX accounts for everyone, though, to keep it simple for the adminstrators is a very good point. I think I will do that otherwise, it will become to hard to manage.
