Calling a Portal Page from a custom Form

Is there a way to call a Portal Page from a Design Studio modified custom form (such as AP20.1). I can use a bookmark to call the Portal Page, but can't figure out how to from within a UI form. I'd like to do it as either a button, action, or related link.

Also, is there a way to pass varibles between a form and a portal page?
