What table and screen is used to store emergency contact information?  I want to upload it via add-ins.  Thank you

I didn't check the table, but the screen is PA12.

This link should be your new best friend You can search by form number and click on that and then it will show you all of the tables updated or referenced by that form. Then you can look at the tables and see information on each of the fields.

Another helper is to place your cursor in the field you are trying to pull and hit at the same time. In the bottom right corner of your IE screen you will see the table name and field name abbreviated.
