LP100 didnt build LP plans

We recently had an issue where approximately 50 new hires did not have an LP31 plan built by the LP100. I have reviewed the following already and am at a standstill as to what to look at next:

1.  PA100 was run in update mode.
2. PA100 and actions had absence management update flag set to yes.  The date of the PA100 was the correct date.
3.  The LP100 was run in update mode and also had the correct date.
4.  The employees are in the employee groups for a PTO plan.

Any thoughts or have you had this happen?
