Supplemental Life Plan

I am trying to test an Employee Voluntary Life plan in our test environment.  This was set up on PR14 directly as opposed to being set up as a benefit plan.  We want to set up Employee, Spouse and Child Life plans for 1/1/14 so I am trying to test this now. 

I added the plan and I am getting an error message each time I try to add the benefit.  In test, I made the effective date 11/1/13.  What am I missing?

We enter our coverage amount for supplemental life insurance in the Cov, Pay Prd field -

My screen has that Cov Pay Prd column greyed out.  I am in a test environment and I'm wondering if it's not set up properly.
