Design Studio trigger won't create work unit

I've inherited a Process Flow that is triggered by a Design Studio lawson xml for an online form (FB40). The xml has a WFWK embedded. We made a change to the development
flow to use a new Design studio xml for the Inbasket (populated FB40 screen). Now the flow refuses to trigger. The online form is checked in Design Studio to be used as the default form. All services and config in Admin screen mirror production (which is still working OK).  What are the usual culprits for this type of activity? Where else can I troubleshoot this? Thanks for everyone's input on this one!

Which version of the FB40 form did you set as the default presentation?

David - thanks for your reply! Not sure where I'm looking to get the exact version, but I see  "XMLGenBy="xscrgen 9.0.1.3.608 2009-10-21 04:00:00 (200811)" in my Deisgn Studio xml fb40 form. I know that we are currently on 9.0.1 MSP6. Do you think my form is outdated?

Did you try initiating the WU manually by tryping the Service Name, Citeria etc in WFWK ? That may provide some clues.

What I meant was, which of two different FB40 forms that you created was set to the default presentation. Sorry for the confusion.
