Ok, since we can't create multiple item masters within a company, how about this. Can we create another company, say 9999, and create an item master within that company just for our pharmacy. We would create a warehouse for each of our remote locations and do a warehouse transfer when sending the remote loc a resupply order. Since it's just a transfer between warehouses it would stay on the books and not be expensed?

That logic sounds correct. You would just need to make sure that the correct item group would be selected in IC01 and your companies are set up correctly in IC01.

There may be financial reporting issues with creating a new company just to accomodate your pharmacy and remote locations. I see no reason why you couldn't add those locations to IC02 within the current company and add the pharmacy items to the primary item master. Then you can add the items to the IC12 for the Pharmacy and remote locations and tie the related supply contracts to those locations. Less complicated that way. And if you're concerned about non-pharmacy staff being able to order pharmaceuticals you could secure those items by Purchasing Class.
