Also-

I have never seen a separate table for plan types only - Where does that info reside? do they reside on the same table as plan categories..

For HR/benefits look at BENEFIT table. How do you define plan and categories? In Lawson plans are like Health, Life etc and categories like vision, dental, supplemental life etc.

They are hard-coded.
