PA52 Upload Template?

Does anyone have an upload template for the PA52 Screen?  We are trying to upload just email addresses.  When we run the upload utility, it appears that they are created successfuly but when I run the PA100 report, they don't show up. 

All I can think is there is one of those "hidden" upload fields.  Any help would be greatly appreciated!

I have one you can use that works with one upload. I will be happy to send it to you if you send me your em address. My em address is mgyurisin@summithealth.org.

I understand others use the method of uploading twice, once as an add and then as a change. You could try that and see if it resolves your issue.

Make sure you map the field number (e.g. F14-PAT-FLD-NBR-111). That will eliminate the need for add then change.
