HR04_Type "LO"

    Using Excel Addins I am looking through PCODESDTL and see a field called FACILITY-NAME however, when looking through HR04 (Type=LO) I do not find that field.  Does anyone know where FACILITY-NAME exists in the application?

      Type=PC (Primary Care Physician) -> HR80.9
